** Responsibilities
* ** Record a patient’s medical history accurately
* Take and record measurements of blood pressure, temperature, heart rate etc.
* Observe patients under treatment to identify progress, side-effects of medications etc.
* Monitor patients’ condition including fluid intake and output and compose patient charts
* Assist ailing patients in daily necessary activities such as eating, bathing etc.
* Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
* Collect samples for lab testing as assigned
* Provide emotional and psychological support when needed
* Communicate with patients’ family or friends to provide advice, comfort and release instructions

** Qualifications
* ** Graduate of an accredited Practical Nursing program.
* Current, unrestricted LPN/LVN license in the state of practice.
* Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
* Experience providing direct patient care to geriatric and long-term care populations.
* Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
* Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.
* Experience with electronic health records (EHRs) and clinical documentation.
* Knowledge of infection prevention and control practices.
* Strong assessment, communication, and critical-thinking skills.
* Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations.
* Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
* Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.
* Current CPR/BLS certification.
