Fondation Soins Lausanne recherche un infirmier référent (H/F) pour intervenir sur le lieu de vie des bénéficiaires, en autonomie et en collaboration étroite avec les proches et les professionnels du réseau.
Le poste, à 70–100% en CDI, propose un travail en journée avec quelques week-ends et une prise en charge globale du patient à domicile, avec des missions d’évaluation, de conseil et de soins infirmiers.
