Reporting to a Continuing Care Manager, the Client Service Assistant (CSA) supports the interprofessional case management team in providing an array of services/programs offered by Nova Scotia Health Continuing Care and the Department of Seniors and Long-Term Care. The CSA provides administrative support for case management functions and works with assigned clients identified as needing additional information and resources to succeed with Continuing Care programs.
The CSA meets with clients and families in community, office settings, and via virtual meeting platforms to ensure they have the information and/or documents required to ensure smooth access to continuing care resources.
Duties of the CSA include but are not limited to:
- Meet with assigned clients and their support people to explain program requirements and assist them in completing Continuing Care forms, documents, and agreements
- Assist marginalized clients with arrangements and organization of documentation, services/supports, and their immediate environment to support the successful delivery of Continuing Care home care services
- Assist other members of the case management team and other Continuing Care team members by providing them with information on available community resources relevant to their client situations
- Take action to liaise promptly with relevant team member(s) when clients require escalated case management services of another team member
- Respond to client-specific inquiries from service providers
- Share documents and information to/from service providers or clients with members of the case management team
- Enter data in health record as directed by a Care Coordinator or Case Worker
- Support the work performed by the interprofessional collaborative case management team
We would love to hear from you if you have the following:
- Completion of a two-year college program in health and human services or related field
- Minimum one (1) year previous, related experience
- Current Emergency First Aid and CPR Certification
- Education and/or experience in dementia care an asset
- Experience working independently in client homes or community settings
- Knowledge of Continuing Care services and programs
- Intermediate computer knowledge and proficient use of Microsoft Office suite, SharePoint, Adobe, desktop faxing
- Excellent communication and customer service skills
- Demonstrated ability to take initiative to resolve issues within the scope of their role using critical thinking skills
- Demonstrated knowledge of social and health supports in the community and within NSH
- Understanding of financial documents and processes such as income taxes, bank statements, payment reconciliations, etc.
- Appreciation for collaborative teamwork and desire to be an effective team member
- Valid Nova Scotia Driver’s License and access to a reliable vehicle as travel is required
- Competencies in other languages an asset, French preferred

Hours- Permanent full-time, 70 hours bi-weekly
- Hours of work may include days, evenings, weekends, and holidays
$28.80 - $31.77 hourly
