• Provide social work services in a primary‑care clinical setting as part of an interprofessional care team.
• Participate in care‑plan development by assessing social determinants of health and planning interventions to address barriers to recovery and adaptation.
• Utilize a variety of treatment modalities, including therapeutic counseling, for clients, caregivers, and families.
• Cooperate and collaborate with Primary Care Team members to deliver quality, client‑centered care.
• Promote professional education and advanced knowledge through research, evaluations, and quality‑improvement activities related to patient care.
• Provide education, leadership, and mentorship to students as a preceptor.
• Facilitate access to community support services, make referrals, and provide input and feedback as needed.
• Promote effective communication among Primary Care Providers, physician specialists, patients, caregivers, and families.
• Develop support components for care plans that maximize patient inclusion, collaboration, and communication with contracted support services, community agencies, and other resources.
• Promote patient independence and self‑advocacy.
• Review patient records to monitor and evaluate care and treatment outcomes.
• Document all patient encounters and interactions and communicate information to the interdisciplinary team as required.
• Use computerized systems for patient records, including entering demographics, histories, and charts in accordance with policies and professional standards.
• Maintain professional currency by participating in relevant educational activities and continuing professional development.
• Provide evidence‑based care by reviewing literature and attending workshops and education sessions.
• Demonstrate initiative in identifying community resources and local partnerships that address patient needs.
• Commit to SHN’s values, Code of Conduct, and comprehensive safety programs, including staff, patient, and environmental safety.
• Engage in ethical conduct and respect the diversity of the patient and staff population.
• Use protective equipment as supplied and report hazards to the appropriate channels.

• Master of Social Work (MSW) degree.
• Registration with the Ontario College of Social Workers and Social Service Workers.
• Experience in community or primary‑care settings.
• Excellent crisis intervention skills.
• Strong focus on psychosocial assessment and discharge planning.
• Proficiency in assessment skills and familiarity with community services.
• Ability and interest in linking patients to community resources.
• Flexibility and strong teamwork orientation.
• Excellent interpersonal and communication skills (verbal & written).
• Judgment when relating to patients, families, co‑workers, and the interdisciplinary team.
• Commitment to SHN’s Code of Conduct and health & safety policy.
• Ability to utilize protective equipment and report safety hazards.
• Excellent work performance and attendance record.
• Asset:
