The ideal candidate for this position holds a university degree in Human Services or equivalent experience and is at least 18 years old. You will provide personal care, assist in decision-making, and follow Individual Support Plans, all while supporting clients' active participation in community life.
Key Responsibilities:
• Implement Individual Support Plans to help achieve client goals
• Advocate for individuals and families with outside agencies
• Provide personal care and support in daily activities
• Maintain accurate documentation and records for each client
• Supervise recreational and cultural activities in the community
Requirements:
• Must hold a relevant Human Services degree or certification
• Minimum of 2000 hours of related experience preferred
• Valid driver's license and insurance
• Ability to manage change and build relationships
• Willingness to obtain required certifications and training
