Position: Nurse Practitioner >

The Nurse Practitioner (NP) is an integral member of our Family Health Team, working within the full scope of practice to provide primary health care services to patients across the lifespan. The NP will focus on health promotion, disease prevention, chronic disease management, and episodic acute care, while collaborating closely with physicians and other interdisciplinary health professionals. There are opportunities for directly attaching patients as MRP, or shared-care with an existing primary care provider.

The GWFHT is a clinical teaching site for the University of Toronto Temerty Faculty of Medicine.

Job Type: Full time or Part time. Work hours can include weekdays and rotational weekends.

Pay range : $120,000 – $130,000 annually

Duties and Responsibilities
  • Provide evidence-based medication management and clinical consultation to patients and healthcare providers.
  • Conduct comprehensive medication reviews to assess therapeutic effectiveness, identify drug-related problems, and recommend appropriate changes
    Prescribe within the scope of practice and provide counseling on the purpose, benefits, side effects, and safe use of prescribed and over-the-counter medications, supplements, and herbal products.
  • Support chronic disease management programs (e.g., diabetes, hypertension, hyperlipidemia) and lifestyle management programs.
  • Collaborate with the interprofessional team to develop individualized care plans tailored to patients’ health goals.
  • Participate in program planning, development, and quality improvement initiatives within the Family Health Team.
  • Maintain accurate documentation in EMR systems and other organizational records
  • Produce reports required for patient care, organizational reporting, and quality improvement.
  • Design, manage, and deliver health programs and educational sessions for patients and staff, including presentations and website articles
  • Provide business consultation for establishing and managing a community pharmacy
  • Communicate effectively and professionally with patients, families, and healthcare team members.
  • Participate in intake, triage, and chronic disease management services
  • Maintain clinic spaces, ensure compliance with Policies, Procedures, and Code of Conduct.
  • Be willing to expand scope of work and train others
Education and Qualifications
  • Current registration in the Extended Class (RN-EC) with the College of Nurses of Ontario (CNO).
  • Primary Care Nurse Practitioner Certificate
  • Member in good standing with the Nurse Practitioners’ Association of Ontario (NPAO) (preferred).
  • Valid CPR HCP certification.
  • Minimum of 2 years’ experience in primary care, community health, or a related clinical setting (new graduates with strong interest in primary care will be considered).
  • Experience with pediatrics, adults, seniors care, chronic disease management, women’s health
  • Demonstrated ability to work effectively within an interdisciplinary team.
  • Excellent clinical assessment, diagnostic, and decision-making skills.
  • Strong communication and patient engagement skills.
  • Strong interpersonal, organizational, and multitasking skills; detail-oriented and team-focused.
  • Ability to take direction from multiple sources (physicians, nurses, team leads).
  • Flexible availability, including some evenings and Saturdays.
  • Able to work across multiple clinic locations within North York
  • Proficiency with electronic medical records (EMRs), computer softwares, and comfort with virtual care platforms.
  • Fluent in English, additional languages reflecting the community (e.g., Cantonese, Mandarin, Farsi, Korean and Tagalog) are an asset.
  • Valid vulnerable sector police record check completed within the last year.
