You will support both locations Monday to Friday, 8 AM to 5 PM, driving behavioral health initiatives for vulnerable populations. This role focuses on psych evaluations, treatment, and education on care strategies for mental health staff. Join a dedicated mission-driven team that aims to improve health outcomes for seniors facing complex behavioral health challenges.
Key Responsibilities:
• Provide psychiatric evaluations and counseling for participants
• Collaborate on care plans with the Interdisciplinary Team
• Educate team members on behavioral health assessments
• Coordinate with community behavioral health specialists
• Evaluate and follow up on inpatient psychiatric participants
Requirements:
• Graduate of accredited Psychiatric Nurse Practitioner program
• 3+ years in behavioral health with elderly populations
• Active Psychiatric Nurse Practitioner License
• State CDS and DEA certifications
• Strong clinical and collaborative skills
Deliver exceptional psychiatric support to enhance health outcomes in underserved communities.
