Home Care Coordinator

Position: Home Care Coordinator at Habitat Health

In this essential role, you will be a key member of the Interdisciplinary Team (IDT) responsible for assessing participant needs and coordinating home care services. Your expertise will drive the development of individualized care plans to create a safe and supportive environment age with participants, families, and caregivers to bridge service gaps and enhance the quality of life for our clients.

Key Responsibilities:

• Conduct in-home assessments for comprehensive care evaluations

• Collaborate with team members on care plan development

• Monitor and renew home care services authorizations

• Provide home safety evaluations and recommendations

• Maintain compliance with electronic documentation standards

Requirements:

• Active Licensed Vocational Nurse (LVN) license required

• Minimum of two years clinical experience in healthcare

• Knowledge of care planning and clinical assessments

• Strong communication and interpersonal skills

• Proficient in electronic health records systems
