The Oncology Nurse Navigator provides vital support throughout the patient experience, from diagnosis to survivorship, ensuring effective care coordination. You will engage closely with interdisciplinary teams to address both clinical and emotional patient needs. Through your expertise in oncology, you will advocate for patients and guide them to necessary resources and clinical services.
Key Responsibilities:
• Ensure compliance with clinical care pathways and guidelines
• Assess and educate patients on treatment options and trials
• Collaborate with social workers to resolve care barriers
• Facilitate team communication and patient engagement
• Screen for distress and connect patients with support services
Requirements:
• Nursing degree; BSN required, MSN preferred
• At least 5 years in nursing, 2-3 in oncology
• Experience in nursing navigation or case management
• Strong problem-solving and critical-thinking abilities
• OCN certification preferred
