You will play a crucial role in our Community Visiting Nursing Program, bringing holistic nursing practice to patients in their homes. As a qualified RN or RPN, you will collaborate with health professionals to establish and administer effective care plans that improve patient outcomes. Advocacy and patient safety are key components of this role.
Key Responsibilities:
• Develop individualized care plans with patients and families
• Provide nursing interventions according to established guidelines
• Evaluate treatment efficacy and make necessary adjustments
• Advocate for patient rights within the healthcare system
• Ensure patient safety and quality care in home settings
Requirements:
• Registered Nurse or Registered Practical Nurse in Ontario
• WOCC or IIWCC certification required
• Minimum one year of community nursing experience
• Strong communication and crisis management skills
• Reliable vehicle and valid driver’s license
