Base pay range$82,000.00/yr - $89,000.00/yr
$4,000.00 Sign-On Bonus-Valid from 12/3/2025-12/31/2025
The bonus will be paid in two installments:
- $2,000.00 after 6 months of continuous employment
- $2,000.00 after one year of continuous employment
The Hospice RN Case Manager provides compassionate, patient-centered nursing care focused on supporting comfort, dignity, and quality of life for individuals receiving end-of-life services in the home setting. This role manages an assigned caseload and ensures each patient’s individualized hospice plan of care is developed, implemented, and evaluated according to VNA standards, clinical best practices, and state and federal regulations.
Essential Duties and Responsibilities- Complete comprehensive nursing assessments and develop individualized hospice care plans in partnership with the interdisciplinary team (IDT).
- Provide skilled hospice nursing services, including symptom and pain management, medication management, wound care, and comfort-focused interventions.
- Coordinate care with physicians, social workers, chaplains, home health aides, therapists, and other team members to ensure seamless support for patients and families.
- Monitor changes in condition and revise care plans promptly.
- Provide ongoing education and emotional support to patients, caregivers, and families.
- Maintain timely and accurate documentation in accordance with VNA policy and Medicare regulations.
- Complete or learn to complete hospice-specific OASIS assessments accurately.
- Participate in IDT meetings, case conferences, and on-call rotation.
- Uphold privacy, safety, and infection control standards.
- Provide leadership and guidance to LPNs, Home Health Aides, and field staff.
- Current Registered Nurse (RN) license in Kansas.
- Minimum one year of nursing experience; hospice or home health preferred.
- Current CPR certification.
- Experience with symptom management and wound care preferred.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong critical thinking and communication skills.
- Proficient in EHR documentation.
- Frequent travel within service area.
- Ability to lift up to 50 pounds.
- Exposure to diverse home environments and end-of-life situations.
