Licensed Practical Nurse (LPN) – Part-Time

Join Douglas County Visiting Nurses as an LPN, providing skilled nursing care at patients’ homes. The LPN travels to client residences, maintains electronic medical records, and coordinates care under RN supervision, operating within federal and state regulations.

Responsibilities

• Assess and document patient/client needs; observe condition at each visit; report changes to RN; follow up with physician as directed.
• Provide direct skilled nursing care, either directly or through RN supervision.
• Educate and support patient/client, family or caregivers.
• Maintain accurate charting and records per agency policy and regulations.
• Coordinate services and visits with patients/families and home health team.
• Know payer source guidelines.

Qualifications

• LPN in good standing with licensing agency; meet federal state licensure requirements.
• Current CPR certification or ability to obtain within 90 days of hire.

Preferred Qualifications

• Home Health experience.
• Education in management of chronic diseases.

Experience

• Basic knowledge of EMR (electronic medical records) and email functions.

Visiting Nurses requires a criminal background check and motor vehicle record (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
