Licensed Practical Nurse (LPN) – Part‑Time

Douglas County Visiting Nurses is seeking an LPN to travel to patients’ homes in Johnson County, KS, to provide skilled nursing care as part of a home‑health team under RN supervision.

• $1,000 Sign‑On Bonus paid in two installments: $500 after 6 months and $500 after 1 year of continuous employment.

• Assess and document patient/client needs and observe patient’s condition at each visit; report changes and concerns in a timely manner to RN and follow up with the attending physician as directed.
• Provide direct quality skilled nursing care, either directly or under RN supervision.
• Provide education, support and teaching of care to the patient/client, family or other caregivers.
• Maintain charting and records as required by agency policy and federal and state regulations.
• Coordinate services and visits with patient/clients and/or family caregivers, as well as other members of the home‑health team.
• Be knowledgeable of payer source guidelines.
• Schedule 25–30 hours per week (6 hours per day – 15 visits per week).

• LPN in good standing with the licensing/certification agency; able to meet all federal and state licensure requirements.
• Current CPR certification or ability to obtain within 90 days of hire.

• Home‑health experience.
• Wound care experience.
• Management of chronic diseases education.

• Basic level knowledge of EMR (electronic medical records) and email functions.

• Criminal background check and motor vehicle (MVR) check required for all employees once a conditional offer of employment has been extended.
