Clinical: RN Clinical Supervisor - Per Diem

NO EXPERIENCE REQUIRED! WE ARE WILLING TO TRAIN. Multicultural Home Care, a Modivcare Personal Care Service, is looking for a Per Diem Nurse as a Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient’s home, evaluating the patient’s plan of care and assessing their physical and Psychosocial needs.

• Schedule: Flexible, will do home visits when aides in service.
• Pay rate per visit:
  • HHA Open $60
  • PC Intro $50
  • HM Open $40
  • Quarterly Supervision: $40
• Visiting Client Territory: Lawrence and surrounding areas:
  Lowell, Chelmsford, Andover, N Andover, Methuen, Dracut, Boxford, Rowley, Newburyport, etc.

• Performs on-site supervisory and teaching visits to evaluate caregivers
• Provides care orientation to caregivers on assigned home care cases.
• Participates in the training of new nurses and in‑services training for field staff.
• Performs initial nursing assessments
• Developing a plan of care and makes necessary revisions based upon patient’s condition/needs.
• Document notes ensuring any changes in the service or patient’s condition are reported
• Monitors records for complete and accurate documentation.
• Coordinates visits and communicate changes with the team

• Registered Nurse (RN) prefers, also will consider LPN.
• Feel comfortable driving and performing home visits in the community.
