The Resident Service Coordinator is responsible for the direct coordination of services and supports for residents at an assigned housing site. This role involves identifying and facilitating access to beneficial resources, acting as a liaison between the Housing Authority/Property Manager and residents when appropriate, and working collaboratively with Housing Authority/Property Manager staff to support all residents. This is a full-time, 37.5‑hour‑per‑week position based entirely at the designated housing site.
On‑site presence is essential for fostering community and connecting residents with vital resources.
- Coordinate and facilitate services and activities to address residents’ needs, challenges, and interests. Survey residents routinely to gauge needs and monitor progress in meeting those needs.
- Inform residents about State Home Care services and other publicly funded programs and public benefits. Make referrals and assist residents with securing benefits as appropriate.
- Assist in screening residents for Home Care Program eligibility and help establish care plans in collaboration with assigned Age Span Care Manager(s) and Nurse(s).
- Work closely with community organizations and social service providers to offer programs that support residents in aging in place.
- Maintain accurate, timely documentation in compliance with requirements set by Molina Healthcare, Age Span, and, when applicable, the Housing Authority/Property Manager. Provide regular updates, feedback, and reports to Housing Authority/Property Manager staff to support tenancy preservation.
- Organize regular meetings with residents to address social, recreational, and educational interests and needs. Identify and work to resolve issues affecting residents’ wellbeing.
- Participate in regularly scheduled on‑site interdisciplinary meetings with Age Span staff members and vendor representatives to monitor residents’ needs and services.
- Attend monthly all‑staff and Community Programs meetings and ongoing RSC team meetings.
- Perform other related duties as assigned.
- BA/BS Degree preferred; LSW preferred.
- Knowledge of and experience with housing/social services with elderly/disabled population strongly preferred.
- Knowledge of Housing/Disability laws and regulations helpful.
- Solid written, verbal, interpersonal and team skills.
- Strong computer skills and comfort with Microsoft applications and programs.
- MARSCH (Massachusetts Association of Resident Service Coordinators) Certification helpful but not required.
- SHINE (Serving the Health Insurance Needs of Everyone) certification a plus.
- Must be able to lift and carry up to 30 lbs., as well as perform physical tasks such as standing, walking, bending, and reaching for extended periods.
- Must have valid driver's license and reliable, personal transportation.
37.5 hours per week.
Pay RangeThe pay range for this position is $27.00 - $29.00/hour.
