Sage Group Health Care Staffing is seeking a Hospice RN CM or a 13-week travel/contract position in Lawrenceburg, TN. Accepting both local and traveler candidates.

Pay Rate:
Range $2000 to $2080 Weekly Gross pay based on a 40-hr work week ( Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health, Hospice
Shift timings: 8 Hours;
Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, with back up on call
Position Requirements:
- Registered Nurse: 2 years - Required
- Recent/Current Hospice nursing in a Home Health setting: 1 year within the last 3 years
- Required - Home Care Home Base (HCHB) charting, hands-on experience and proficiency: 1 year
- Required - Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated orprovide medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for State of Indiana or Compact
-Required - BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Provide case management services for the assigned patient case load.
- Support pain management andability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
- Empower patients and families toreach healthcare goals by educating them on disease management
- Case management and provide clinical oversight to interdisciplinary teams
- Provide nursing care and services within the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status
- Document and complete charting for all patient visits within 24 hours.
