Job Summary:
We are seeking a compassionate and skilled Nurse (LPN or RN) to join our dynamic healthcare team at Countryside Post-Acute and Rehabilitation Center. • Provide direct patient care to residents, including vital sign checks, medication administration, and wound care
• Develop and implement individualized care plans with input from interdisciplinary team members
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
• Maintain accurate and up-to-date records of patient interactions and care activities
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
• Demonstrate a commitment to excellent customer service and patient-centered care

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
