Responsibilities
  • Perform all work in a manner consistent with NYAP’s mission, values, and philosophies.
  • Provide children with a positive and appropriate medical environment.
  • Ensure confidentiality of health records and adhere to NYAP, state, and funder PHI guidelines.
  • Obtain MOUs with medical, dental, and mental health providers.
  • Participate in intakes and discharges, including COVID-19 testing and documentation.
  • Continuously assess children’s medical needs and coordinate care per NYAP policies.
  • Collaborate with cross-functional team and providers to schedule appointments, prepare TARs, transport children, and obtain clearances.
  • Report medical emergencies via SIR/CLE process.
  • Escalate medical concerns to supervisor, team, providers, or DHUC for guidance.
  • Support foster parents with medical education and documentation.
  • Collect and upload medical forms and records to appropriate portals.
  • Maintain and submit all required documentation on time.
  • Communicate regularly with care teams and meet quality improvement targets.
  • Maintain medicine cabinets and first aid kits.
  • Provide-service training on medication administration, COVID-19 testing, CPR, and first aid.
  • Maintain Red Cross CPR/First Aid trainer certifications.
  • Assist and support program leadership during audits.
  • Perform other duties as requested.
Minimum Qualifications
  • LPN/LVN or higher health qualification (e.g., RN with bachelor’s) OR bachelor’s degree in relevant field.
  • Bilingual fluency in English and Spanish required.
  • Minimum 1 year of employment in public health or health care with experience in care coordination, health administration, or direct clinical care.
  • Minimum 1 year experience working in child welfare with children/adolescents.
  • Valid driver’s license, excellent driving record, safe personal vehicle, current auto insurance.
  • Ability to comply with agency training requirements.
  • Proficient with desktop, laptop, smartphone, tablet, printer, fax, copier, and software (word, spreadsheet, database).
Other Skills
  • Experience scheduling medical appointments.
  • Experience documenting medical appointments.
  • Understanding needs of immigrant population and providing support.
  • Ability to maintain liaisons with community health providers and expand network.
  • Willingness to work non-traditional hours and on-call phone consultations.
Physical Demands
  • Manual dexterity, tactile, visual, and audio acuity.
  • Repetitive motion, prolonged sitting and standing, sustained visual and mental focus.
  • Occasional lifting up to 25 pounds, bending, pulling, carrying.
  • Ability to travel frequently and drive vehicle for extended periods.
