MeSun Hospice is seeking a compassionate, dedicated, and self-directed Full-Time RN Case Manager to join our hospice team.
This position is ideal for an RN who values compassionate, patient-centered end-of-life care and enjoys working both independently and as part of an interdisciplinary team.
Key ResponsibilitiesCase Management: Manage a manageable caseload. Educate patients and families regarding hospice philosophy, disease progression, medications, symptom management, and what to expect throughout the hospice process.
Patient Visits: Conduct routine patient visits based on each patient's individualized plan of care and clinical needs.
Minimal Travel: Most patients are clustered in nearby areas, significantly reducing travel time.
Qualifications- Active Georgia Registered Nurse (RN) license
- Required - Valid driver's license and reliable transportation – Required
- Bilingual in Korean and English (Highly Preferred) to effectively communicate with our Korean-speaking patients and families.
- Strong nursing assessment, communication, organization, and clinical judgment skills
Full-Time
- Field visits combined with case management, care coordination, and documentation
