As a Licensed Practice Nurse (LPN), you will deliver skilled, compassionate, one‑on‑one, patient‑centered care directly in patients' homes. Compensation is based on experience, shift type, and market factors. The listed range reflects entry‑level to highly experienced applicants.

• Provide skilled, in‑home nursing care to assigned patients
• Develop and follow individualized care plans
• Accurately document patient care, observations, and updates
• Collaborate with interdisciplinary care teams
• Maintain compliance with DOL, CMS, and state regulatory requirements

• Current LPN license in the state of Utah
• Current CPR certification
• Valid driver's license and reliable, insured vehicle
• Home health experience preferred but not required
• Must pass pre‑employment screenings
