Base pay: $77,600 – $106,600 per year. Additional compensation per visit: $49.00 – $69.00.

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, provide necessary follow‑up as directed by the Clinical Manager.
• Confer with physicians in developing the initial plan of treatment based on orders and initial assessment; manage and evaluate the care plan and teach the patient under Clinical Manager supervision, revising the plan as required by policy/regulation.
• Coordinate appropriate care among healthcare personnel (physical therapists, occupational therapists, home health aides, external providers).
• Report patient care, condition, and progress to the patient’s physician and Clinical Manager continuously.
• Implement patient care plan with the patient and family to achieve optimal needs resolution.
• Coordinate/oversee/supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written personal‑care instructions that reflect the current plan of care and monitor documentation completion.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing required clinical documentation.
• Prepare medical documentation for all patients, including case conferences, patient contacts, medication order changes, re‑certifications, progress updates, and care‑plan changes; prepare visit/shift reports and updates of patient records.

• Diploma, Associate, or Bachelor Degree in Nursing.
• Minimum of one year nursing experience preferred.
• Strong medical‑surgical, ICU, ER, or acute experience.
• Home Health experience a plus.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
