Job Title: Hospice Registered Nurse (RN)
Day 5x8-Hour (08:00 - 17:00)

Position Summary:
This center is needing a Hospice RN with Homecare Homebase (HCHB) experience who can hit the ground running. The RN will be assigned a tablet, see patients the same day each week, and participate in the on-call rotation.

Work Schedule:

• On-call rotation

• Monday – Friday (8:00 AM – 5:00 PM)

Charting Requirements:

• Homecare Homebase (HCHB) & OASIS

• Must complete charting while at patient home

Average Patient Load: 4–6 patients per day

Points / Hours:

• On average 30 points per week (equivalent to ~40 hours per week)

Guaranteed Hours: Varies from facility to facility

Types of Cases:

• In-home patient care

• Interdisciplinary care coordination

• Plan of Care development and oversight

• Geriatric population

EMR: Homecare Homebase (HCHB)

Experience Required:

• Minimum 1 year Home Health experience

• 2 years' experience overall as a clinician
