Pay Rate: $2600 Weekly Gross Pay based on a 40-hour work week ( Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Position Requirements:

• Current Physical Therapist experience: 2+ years
  - Required
• Home Health experience
  - Required
• Prior travel experience:
  Highly preferred

Education, License and Certification Requirements:

• PT License for State of Illinois - Required
• BLS Certification from AHA
  - Required

Job Description:

• Provide all aspects of Physical Therapy Services to patients in a home health setting.
• Plan and execute rehabilitation programs to help patients return to ahigher level of physical function.
• Utilize a variety of skills including, but not limited to, therapeutic exercise, neuromuscular re-education, physical modalities, gait training, andmanual techniques.
• Demonstrate effective communication and strong interpersonal skills to motivate patients and provide compassionate care.
• Fleet car will be provided for the therapist to drive.
