? Hospice Registered Nurse (RN) Case Manager Full-Time | Monday–Friday, 8a–5p |Serving Patients Across Leawood & Surrounding Communities

? What You’ll Do
• Conduct comprehensive patient assessments and create personalized care plans
• Collaborate with the interdisciplinary hospice team to provide high-quality care
• Educate and support patients, families, and caregivers on symptom management, pain control, and hospice philosophy
• Monitor patient condition and communicate changes to the care team and physician
• Offer emotional and spiritual support at end-of-life
• Provide care in home settings, assisted living, and skilled nursing facilities
? What You’ll Need
• Graduate of an accredited nursing program (Diploma, ADN, or BSN)
• Active RN license in the state of Missouri
• 1+ year of clinical experience (hospice or home health preferred)
• Reliable transportation, valid driver’s license, and current auto insurance
• Highly preferred to have HOS or SHH experience
• Physical ability to sit, stand, bend, lift, and move intermittently (up to 50–100 lbs)
