Occupational Therapist – Ledbury Intermediate Care Unit, 12‑month fixed term secondment. This role combines work in the intermediate care unit and in patients’ homes as part of the discharge assessment pathway. The aim is to optimise function and independence in physical, emotional and social aspects, facilitate discharge, prevent further admissions, and reduce inpatient length of stay, in line with care‑closer‑to‑home principles.
MainResponsibilities
- Provide high‑quality occupational therapy assessments and direct rehabilitation across a range of conditions.
- Work autonomously on the unit and in the community, liaising with health and social care professionals to support discharge planning.
- Triage and prioritise cases, manage caseload effectively, and maintain organised records.
- Supervise team members remotely, foster a positive team culture, and demonstrate passion for community rehabilitation.
- Signpost and refer patients to wider community services, negotiating with ASC, Neuro, etc.
- Maintain professional CPD and ensure compliance with UK professional registration requirements.
You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and will require disclosure to the Disclosure and Barring Service.
Person SpecificationEducation & Qualifications
- Degree in Occupational Therapy
- HCPC Registration
- Post‑graduate specialist qualification or significant post‑graduate experience
Skills, Knowledge & Abilities
- Broad and specialist knowledge of relevant patient conditions and therapy interventions
- Demonstration of core and specialist therapy skills
- Evidence of ongoing CPD in the relevant area
- Experience of multi‑disciplinary communication, assessments and record keeping
- Computer skills including word processing and e‑mail
Experience
- NHS or equivalent experience in inpatient, acute, community or other health services
- Experience of using/fitting adaptive equipment
