Overview

Wiltshire Council is looking for an experienced Occupational Therapist to join the Reablement Service. The role involves working closely with multidisciplinary teams in Great Western Hospital (GWH) and Royal United Hospital (RUH) to support timely and effective hospital discharges into reablement.

Title: Occupational Therapist – Reablement Service

Location: Wiltshire

Pay: £34.92 per hour

Key responsibilities include:

• Collaborate with multidisciplinary teams to plan and execute effective patient discharge strategies.
• Assess patients’ functional needs and develop individualized reablement plans.
• Document and report progress to hospital and community care teams.

• Qualified Occupational Therapist.
• Experience working in a fast‑paced, busy environment.
• Proficiency in working with acute hospital teams and discharge planning.

• DBS check processed and paid for by Eden Brown Synergy.
