We are seeking a dedicated and experienced Physiotherapist to join our client’s Reablement team in Leeds, West Yorkshire. This role involves working closely with individuals to support their recovery and independence following illness or hospital discharge. The successful candidate will play a key role in delivering short-term, goal-focused interventions that promote mobility, function, and overall well‑being.
Key Responsibilities
In this role, you will assess patients’ physical abilities, develop personalised reablement plans, and deliver physiotherapy interventions aimed at restoring independence. You will work collaboratively with occupational therapists, support workers, and other healthcare professionals to ensure a holistic and person‑centred approach to care.
Qualifications and
Experience:
- HCPC registration
- Degree in Physiotherapy
- Previous experience in a reablement or community setting is desirable
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to promoting patient independence and recovery
