At Leeds Community Healthcare, we believe in providing exceptional care that goes beyond the ordinary, and we want to appoint Community Nursing Assistants to help us achieve that vision.
Main duties of the jobThis is an exciting and varied role that incorporates many different aspects of patient care delivered to a wide range of patients.
- You will support our palliative patients, providing one‑to‑one care in their own homes in their last weeks of life. When working with a palliative patient you will be based in the patient home overnight.
- You will support the Registered Nurses, responding to intervention call‑outs overnight or to work alongside a colleague and visit patients overnight for a planned intervention.
- You will also support the Virtual Ward (Frailty) providing coordinated rapid care to people in their home who are experiencing an acute medical episode.
- It would be expected that you are available to work on a rota over 7 nights per week, 365 days per year. Rotas would be issued with a minimum of 4 weeks notice.
- Your hours of work will be 21:45‑07:15.
- The contract is for a minimum of 18 hours (2 nights) up to a maximum of 36 hours (4 nights). Hours can be discussed prior to application or at interview as required.

Employment conditionsThis role is based in the UK. Employment is conditional on confirmation of the right to work in the UK – either as a UK or Irish citizen, under the EU settlement scheme or having secured any other relevant work visa. You must have a full driving licence and access to a vehicle suitable for work purposes as driving citywide is an expectation of the role.
