Occupational Therapy Assistant Practitioner
The CDAT service has developed in the last year to offer a same day therapy service. The therapy offer will be available to support the team with any OT specific assessments/tasks. We have identified that there is a need for a Therapy Assistant Practitioner (TAP) to support the OT/ registered therapist (RT) role within the CDAT service. The RT will carry out mobility/transfer assessments, complete functional assessments where needed on the Acute Admissions wards, including but not limited to kitchen assessments, personal care assessments and cognitive assessments.
The RT will carry out home/environmental visits where needed and if appropriate assess the patient in their own environment. The role of the RT and TAP within CDAT will aim to embed the pull model at the front door to reduce the number of admissions into LTHT. This is a new role and will require personal drive and commitment to make this work.
The ideal candidate will work in partnership with us as we develop this service and improve the overall offer of CDAT services at the front door. You will be expected to actively engage in the programme and uphold the Trusts vision and values. You will be required to attend a series of training courses, demonstrate reflective practice, and participate in meetings with your supporting therapist and Clinical Therapy Lead to review your progress.

You will be required to attend a series of training courses, demonstrate reflective practice, and participate in meetings with your supporting therapist and Clinical Therapy Lead to review your progress.
TAPs will see a range of patients who often have several health and social care needs and require therapeutic intervention and rehabilitation to maximise their independence. You will assume responsibility for your own caseload within the boundaries of your practice, under the guidance of a registered therapist. The registered therapist will delegate responsibilities to the TAP and is clinically accountable for the patients care.
The main duties of the role may include but are not limited to the following:
- Mobility practice
- Upper and lower limb exercise programmes
- Stair practice
- Functional assessments- kitchen and wash/dress assessments
- Management of the peripheral store. Including keeping track of equipment issued and restocked
- Collate patient data using the ED patient tracker updating patient outcomes when known
- Complete home visits to conduct environmental assessments or joint home visits with the OT
- Deliver and fit basic transfer equipment
- Complete initial assessment screenings and contact NOK for collateral history
- Working towards goals set out by therapists
- Providing additional assistance with patients who require assistance of 2 for mobility or transfers
