We have an exciting opportunity for a Band 6 Occupational Therapist who is dynamic and passionate about stroke rehabilitation to join our supportive and caring team.

The Community Stroke Rehabilitation Team offers specialist multi-disciplinary rehabilitation to adults who have had a stroke in a community setting.

You will work closely with other professionals such as physiotherapists, speech and language therapists, psychologists, dietitians, nurses, therapy assistant practitioners and rehabilitation assistants.

The service provides goal‑focused, time‑limited rehabilitation interventions in patients’ home environments and in other settings as appropriate. The stroke rehab team works closely alongside colleagues from Leeds Teaching Hospital Trust, as well as third‑sector organisations such as the Stroke Association, to support patients along the whole stroke pathway, from hospital to home.

We are implementing a new intensive rehabilitation pathway to support earlier discharge from hospital alongside our current offer. The ideal candidate will have experience of managing people through change and strong leadership skills, as well as previous clinical knowledge relating to stroke rehabilitation and experience of supporting MDT colleagues.

Joining the team will involve designing and implementing this new intensive pathway, supporting staff through change, providing business‑as‑usual services, and line‑managing band 5 occupational therapists and/or unregistered staff within the team.

The successful candidate will have experience working with patients recovering from stroke or other neurological conditions and be familiar with current evidence‑based practice.
