About the role

At Avery Healthcare, we are supporting the people who care for our residents. Recognised for our award‑winning team at Aire View Care Home in Leeds, we seek a warm, enthusiastic, and dedicated Team Leader to support the home management team in delivering high‑quality care.

• Lead the person‑centred care planning process, including implementation, monitoring, monthly evaluations, care plan audits, and action plans.
• Assist with smooth transition of discharges, ensuring documentation, medication, and next‑of‑kin communication.
• Monitor the well‑being of each resident, record changes, and adjust care plans accordingly.
• Practice safe systems of work for tasks, moving and handling residents, assessing risks and safeguarding personal and resident safety.

• NVQ Level 3 (or equivalent) in care for care homes.
• At least two years of senior experience working with residents.
• Effective verbal and written communication skills.
• Ability to lead staff and act responsibly in the absence of the manager.

• DBS disclosure check – cost to be met by Avery Healthcare.
• Proof of eligibility to work in the UK.

£15.00 per hour.
