About the role
At Avery Healthcare, we are supporting the people who care for our residents. Recognised for our award‑winning team at Aire View Care Home in Leeds, we seek a warm, enthusiastic, and dedicated Team Leader to support the home management team in delivering high‑quality care.
Key responsibilities- Lead the person‑centred care planning process, including implementation, monitoring, monthly evaluations, care plan audits, and action plans.
- Assist with smooth transition of discharges, ensuring documentation, medication, and next‑of‑kin communication.
- Monitor the well‑being of each resident, record changes, and adjust care plans accordingly.
- Practice safe systems of work for tasks, moving and handling residents, assessing risks and safeguarding personal and resident safety.
Our ideal candidate will embody the values of caring, supportive, honest, respectful, and accountable.
- NVQ Level 3 (or equivalent) in care for care homes.
- At least two years of senior experience working with residents.
- Effective verbal and written communication skills.
- Ability to lead staff and act responsibly in the absence of the manager.
- DBS disclosure check – cost to be met by Avery Healthcare.
- Proof of eligibility to work in the UK.
£15.00 per hour.
