This post will involve working alongside community care bed providers/Recovery Hubs and our hospital-based Carer Support Workers to provide support to carers after someone has been discharged from hospital and specifically to find appropriate permanent care placements or domiciliary packages of care where the person with care needs will be self-funding.

Job Description

Patient Flow Carer Support Worker Key duties and responsibilities
  • To develop a close working relationship with the Community Care Bed Multi-Disciplinary Teams across the city.
  • To provide person-centred support, advice and information to carers, and those with care needs, to support discharge to permanent care (or home with a package of domiciliary care) where the person with care needs will be self-funding this care.
  • To undertake monitoring and evaluation of the service and prepare regular reports on the work undertaken.

This includes our Advice Line, carers support groups, one to one support and support in hospitals. Some of our services are universal – open to all carers – and some are targeted at specific groups of carers. Our services are provided to all communities throughout Leeds.

We work in partnership with others to deliver our service and to influence them to act to benefit unpaid carers. This involves those working in health and social care and employers. We use what unpaid carers tell us about their experiences and what matters to them, to help create change at a local and national level.
