Support Worker to be at the forefront of providing high-quality, enabling care. Individual Outcome and Wellbeing:
Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance. Activity Planning:
Develop flexible, engaging activity plans that promote autonomy and personal growth.
Health and Lifestyle:
Support individuals inmaintaininghealthy lifestyles and managing their health needs.
Communication:
Facilitate effective communication andmaintaindetailed,person-centered records.
Medication Management:
Oversee medication processes and support individuals in self-administration.
Safety and Environment:
Maintaina safe, clean-living environment and adhere to health and safety protocols.
