Dementia Support Worker

Care & Repair Leeds is the Home Improvement Agency for the city, highly regarded for providing a range of services for older people and disabled people to help them to remain living independently in their own homes. As an independent Registered Society with charitable status covering the whole of the Leeds Metropolitan District area, we have been providing services since 1987.

This is an exciting opportunity to join our dedicated Support Worker team, enhancing the skills and expertise of the team by bringing your knowledge of the needs of people living with dementia, their families and carers. You will work closely with our partners, Age UK Leeds and Groundwork, delivering the Leeds City Council commissioned Home Plus Leeds service. You will visit people with dementia in their own homes to assess for interventions and support to make their home more dementia-friendly, improving their health and wellbeing, and their ability to live independently in their own home.

You must have proven experience of working within a casework and/or support worker role within a community setting; knowledge and appreciation of the housing issues experienced by people living with dementia (their families and carers), older people, disabled people and people with health conditions which impact on their ability to live safely in their own home. You will have experience of supporting people with a wide range of needs, communicating sensitively and effectively to identify their needs and arranging for appropriate interventions to be put in place, and/or signposting to other agencies, ensuring a more sustainable outcome for the client.

You will be IT proficient, including the use of Microsoft Office, and be physically able to lift and carry items of falls equipment such as raised toilet seats and bath boards.
