Morley and District PCN is seeking a registered Nurse to join the Proactive Care Team, with a special interest in Frailty and long-term condition management. This new role supports our Neighbourhood Health care model in Morley alongside our proactive community commitments.
You will work within a multidisciplinary team, provide home and community-based care, and contribute to proactive frailty management and individualized care plans.
