We are looking for a compassionate and experienced Home Care Nurse to provide high-quality nursing care for patients in their own homes. The ideal applicant will have outstanding communication skills, good leadership qualities, and the ability to create individualized care plans. This role is paid and for people committed to enhancing health and wellbeing via individualized nursing services.

• Provide complete nursing care to meet the particular needs of patients at their home.
• Develop, execute and periodically review customized care plans in partnership with the patient and his/her family.
• Observe patient health status, provide medication, and perform routine health evaluations.
• Educate and support patients and carers on health management and treatment methods.
• Work with multidisciplinary teams to ensure continuity of care and optimize patient outcomes.
• Ensure all care actions, observations and interventions are accurately documented in compliance with regulatory criteria.
• Set a good example to others in observing high levels of cleanliness, safety, and infection control procedures.
• Provide leadership and advice to junior personnel or carers involved in patient care when appropriate.

• Candidate:
  Woman.
• A nursing qualification or similar experience caring for others.
• Gentle, patient and reliable.
• Ready to work as a live-in caregiver.
• Good English communication skills.
