The LTC Proactive Care Nurse role within York Road PCN is to deliver proactive, evidence-based assessment, management and care planning for patients with complex long-term conditions and frailty. The post holder will operate across the PCN footprint, including patients' homes, care homes and community settings.
You will work with the PCN's most frail and clinically complex patients, undertaking holistic assessments, coordinating care and supporting self-management to reduce hospital admissions.
