What you will do:
- Act as the lead professional/case coordinator for a caseload of young people.
- Complete holistic assessments and develop individualised recovery plans using a strengths-based approach.
- Provide 1:1 recovery-focused support, including motivational interviewing and goal-setting.
- Work closely with families, carers, schools, social care, CAMHS, youth justice, and voluntary services.
- Safeguard young people in line with UK safeguarding policies and procedures.
- Maintain accurate case records and reports in line with GDPR and organisational standards.
- Support young people to access education, training, employment, health, and community services.
