Join an exciting opportunity as an Advanced Adults Social Worker within the Duty Team in Leeds. We are seeking an experienced and motivated social worker to support the East Leeds locality, with a particular focus on the Meanwood area. This role is integral to the smooth operation of the duty service, ensuring timely responses to adult social care needs while working collaboratively with colleagues and partner agencies.

- Manage the duty function of the Meanwood Team alongside another social worker.
- Maintain and update the duty handover to ensure seamless continuity of service.
- Attend multidisciplinary team (MDT) meetings via Microsoft Teams and contribute to coordinated care planning.
- Manage and update the duty calendar, ensuring visits and interventions are scheduled appropriately.
- Provide advice, guidance, and support to adults requiring social care intervention.
- Undertake assessments and contribute to decision‑making processes in line with Adult Social Care legislation and best practice.
- Respond promptly to referrals, enquiries, and urgent situations as part of the Duty Team function.
- Work collaboratively with health professionals, partner agencies, and community organisations to provide effective support.
