Job Summary
We are offering an exciting opportunity to join our Older Persons and Physical Disability Teams as a Level 2 Registered Social Worker based in the Staffordshire Moorlands.
The service supports people of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.
Main DutiesAs a Level 2 Social Worker, you will offer a flexible approach with the ability to forge excellent working relationships with multi‑disciplinary teams and partner agencies. You will be expected to be an experienced practitioner with in‑depth knowledge of safeguarding and complex case management.
Responsibilities- Play a key role to support the delivery of the Personalisation and Inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset based practice, personalisation and social inclusion is embedded and applied effectively.
- Carry out a range of assessments including holistic needs led assessments, work of a more complex nature, care and support planning and reviews, and where appropriate, initiate a multi‑disciplinary assessment.
- Have comprehensive knowledge of the services available to support and promote independence in the home environment.
- Providing strength‑based practice, forging links within local communities to provide an outcome focused, person centred service as we move towards a meaningful integrated care system.
- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or foreign qualification certified by the Professional Social Work regulatory body.
- Current registration with the Professional Social Work regulatory body at all times.
- Approved Mental Health Professional and/or Best Interest Assessor status as per the SWLA Career Pathway.
- Work Based Supervisor and Practice Educator status as per the SWLA Career Pathway.
- Good keyboard/IT skills including word processing, spreadsheets, email and PowerPoint.
- Ongoing commitment to personal and professional development.
- Clear understanding of and commitment to equal opportunities and anti‑discriminatory practice.
- Existing accreditation relating to Practice Educator/Work Based Supervisor.
- Significant post‑qualification experience of working with adults with needs associated to older age, physical health or mental health needs and their carers.

Salary: £38,682 to £46,580 a year. Permanent, full‑time.
