The Family Rapid Re-Housing Case Manager (RRH-CM) will provide shelter case management and housing location services for single and family households in Loudoun County who are experiencing homelessness, seeking housing, and referred for Rapid Re-Housing services. The RRH CM will also provide community-based Case Management services for those clients recently housed. The Case Manager will deliver services that are low-barrier, housing-focused, and align with the Housing First model, in addition to being trauma-informed.
The Case Manager will welcome all clients with dignity and respect and will work to help support each client in achieving their goals and locating and maintaining housing. Day to day responsibilities include conducting family housing assessments upon receipt of a referral, assisting clients in accessing resources and benefits, conducting goal setting, budgeting, planning activities with clients, assisting clients in locating housing, and empowering clients' progress not only in gaining housing but sustaining it.
Once housing is secured, the Case Manager will work to link households with services and resources to help them stabilize in their new housing and neighborhood. This follow-up case management often includes assisting households with budgeting, learning how to be a successful tenant, increasing income, and linkages to other service providers in the community. The work conducted by this position is fast paced and client relationships are often time limited.
The Case Manager must be comfortable forming relationships with landlords and other community partners to ensure clients are able to secure housing as their top priority.
- Provide relational and responsive case management support to each family and empower and support clients in their successful completion of their identified goals
- Review referrals and speak with families to determine program eligibility and assess household needs
- With each assigned family, develop Housing First focused service plans which are individualized to the household's unique housing need; assist each household through the housing location process
- Complete any relevant assessments or referrals needed to support the client's housing
- Once housing is secured, partner with families to outline what supports are needed to address housing barriers and needs, and to develop an agreed upon plan to achieve housing related goals and maintain housing stability
- Assist family in securing needed or requested basic needs items and services such as clothing, employment opportunities, benefits, health resources, employment, childcare, educational resources, etc.
- Work to collect financial information from families as well as formulate a working budget with them
- Act as Housing Locator, communicate and establish working relationships with local landlords to negotiate lease terms and advocate on behalf of clients
- Assist Shelter Case Managers in housing location for their clients
- Assist with developing a rental assistance package for all clients utilizing a variety of funding sources, properly document the distribution of funds, and lead the recertification process if families require additional funds to maintain housing.
- Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees, deployed staff, and involved agencies to ensure clients receive needed services in a timely manner
- Assure that all relevant information pertaining to families is disseminated to all appropriate staff members in a timely manner
- Encourage and support the families to focus their efforts on attaining the objectives specified in their Service Plan
