STARTING SALARY: $1,848.85 bi-weekly. (In accordance with the current spending restrictions, an internal promotion into this position will be filled at the minimum of the pay grade or up to 5% of the employee's current rate, whichever is higher.)

DUTIES AND RESPONSIBILITIES
  • Case Management, Referrals, Recommendations and Assessment
  • Initiate contact with youth, family, victims, and law enforcement when referrals are received by FDJJ.
  • Conduct home and school visits as part of the information collection/family involvement model.
  • Use Motivational Interviewing to engage youth and family and identify needs.
  • Obtain and review collateral information from other agencies involved with the youth and family.
  • Conduct and document FDJJ screening(s) and assessments to identify risk and needs.
  • Facilitate completion of comprehensive assessments and refer youth and family for services regardless of legal status.
  • Document all referrals for services and the acceptance or refusal of services.
  • Formulate case management strategies based on assessments and information gathered.
  • Make service referrals to appropriate providers and follow up actions.
  • Make supervision and treatment recommendations to the State Attorney and other judicial partners.
  • Discuss safety plans to prevent victimization.
  • Complete screening instruments and reports used by the Department and courts.
Court Process
  • Act as DJJ representative during court hearings to support Department recommendations.
  • Advocate with attorneys on Department recommendations based on screening, assessment, intake and supervision.
  • Prepare and maintain court reports, referral letters, and related correspondence.
  • Explain court procedures to youth and families.
  • Provide current information to staff responsible for detention screening and reviews.
  • Testify at hearings when appropriate.
  • Prepare affidavits/petitions for Violation of Probation or any violation that results in a delinquency petition.
  • Request hearings when appropriate and communicate outcomes to all parties.
Case Management, Interventions & Supervision
  • Complete risk and needs assessment for all supervised youth.
  • Conduct face-to-face contacts in home, school, and community settings.
  • Maintain communication based on Department requirements.
  • Assist youth with obtaining mental health and substance abuse evaluations.
  • Follow up with service providers and ensure progress reports are received.
  • Assist youth and family with community resources and services.
  • Assist with school enrollment, attendance, academic performance, and behavior.
  • Arrange life skills groups, counseling, and recreation activities.
  • Provide crisis intervention and follow up on referrals.
  • Work with youth to establish personal goals and provide ongoing support.
  • Assist with transportation coordination to court, school, and residential programs.
  • Monitor specialized caseload and refer for services.
  • Respond to concerns and risks and provide input on interventions.
  • Determine recommended handling of violations and provide appropriate interventions.
  • Conduct staffing when benchmarks and case milestones are achieved.
  • Prepare case transfer or progress summaries within required time frames.
  • Participate in transition and exit planning and provide post-residential referrals.
Records Management
  • Enter all case management activities into JJIS.
  • Upload required forms and documents into JJIS case file.
  • Organize and maintain hard copy or electronic records.
  • Review referral history and other JJIS data for accuracy.
  • Obtain and maintain youth’s photograph in JJIS.
  • Conduct case reviews in JJIS and correct data entries.
  • Act as liaison to the Circuit Data Integrity Officer on JJIS related issues.
Community Collaboration
  • Provide contact information for community-based service providers.
  • Meet and maintain communication with schools, teachers, law enforcement, care providers, and other agencies.
  • Provide input to circuit management on provider community-based service delivery issues.
  • Ensure familiarity with community resources.
  • Schedule work crews and supervise youth participating in group work projects.
  • Act as a liaison with co-workers, community organizations, agency partners, and the court.
