Zorgplein Noord in Leeuwarden zoekt een persoonlijk begeleider voor het ondersteunen van jong volwassenen met psychosociale problematiek. In deze rol help je bewoners om zelfstandig te leren leven, waarbij je zorgplannen opstelt en andere begeleiders aanstuurt.
