Role overview
Leicestershire Partnership NHS Trust is working with Adult Social Care across Leicestershire and Rutland to deliver the Home First service alongside core Community Therapy. This role will support people to remain in their homes during health or social care crises, help them return from hospital quickly, and provide rehabilitation and reablement to restore health and wellbeing.
Responsibilities- Provide effective and efficient therapy services as a Band 6 Physiotherapist.
- Manage your own allocated caseload with initiative, prioritise tasks and work autonomously and as part of a team.
- Demonstrate excellent communication and organisational skills and adapt to a changing, pressurised environment.
- Work with Occupational Therapists and Technical Instructors to carry out comprehensive functional assessments and interventions to enable patients live independently.
- Represent Leicestershire Partnership Trust in the Discharge Hub, completing triage at the point of referral in collaboration with Adult Social Care and University Hospitals of Leicester.
- Support patient flow and discharges from hospitals following Discharge to Assess principles.
- Work synergistically with therapy teams to deliver the best possible outcomes.
- Band 6 Clinical Grade Physiotherapy qualification and registration with the Health and Care Professions Council.
- High level of personal responsibility, resilience, proactivity and motivation.
- Evidence of ability to prioritise, work autonomously and collaborate within a multidisciplinary team.
- Strong communication, organisational and problem‑solving skills.
- Experience or willingness to provide rehabilitation, reablement or core therapy services in community or discharge settings.

DBS disclosure costs will be met by the individual and deducted from salary once started.
