Waythrough is recruiting Discharge & Recovery Coordinators to support the Housing, Health & Wellbeing Hospital Discharge Support Service across County Durham. You will coordinate safe discharges, conduct assessments, and work with hospital teams, Adult Social Care, housing providers and partners to ensure timely access to housing and recovery plans.
