CCH Group is seeking a Care Coordinator to help organise and deliver high-quality home care in the community. You will coordinate referrals, create care worker rotas, and liaise with social workers and GPs to ensure consistent, compassionate care.
Working hours are Monday to Friday 9am–1:30pm with on-call duties on a rota; a driving licence is required. This role offers leadership development within the UK’s largest care provider and access to supportive benefits.
