We would like to offer you the opportunity to actively support and deliver effective patient care in our PCN. You need to be a qualified pharmacist, and it would be desirable if you had primary care experience. If you are an independent prescriber as well that would be great. We are happy to train if you don’t.

This is an exciting time to join the PCN with new services developing to support our patients and practices. The role will suit someone who is forward‑thinking, innovative and enthusiastic. You will need to be adaptable, self‑motivated and organised.

The Clinical Pharmacist will work as part of a multi‑disciplinary team in a patient‑facing role; will take responsibility for areas of chronic disease management within the practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The Clinical Pharmacist will provide primary support to general practice staff with prescription and medication queries, repeat prescription systems, acute prescription requests, and medicines reconciliation on transfer of care. They will address public and social care needs of patients in the GP practice(s).

The Clinical Pharmacist will provide clinical leadership on medicines optimisation, ensure integration with community and hospital pharmacy, and help manage workload to improve patient outcomes and operational efficiencies.

The Clinical Pharmacist will be supported to develop their role to become a non‑medical prescriber.

• Lead patient‑facing long‑term condition clinics (e.g., COPD, asthma) and optimise medicines.
• Conduct patient‑facing clinical medication reviews, including for care homes and domiciliary patients, producing recommendations for senior staff.
• Manage common/minor/self‑limiting ailments, signpost to community pharmacy or refer to GPs when appropriate.
• Deliver patient‑facing clinics and telephone support for medicine queries.
• Answer medicine‑related enquiries from staff and patients; suggest solutions and provide follow‑up.
• Audit and review medicines associated with unplanned hospital admissions; implement changes to reduce high‑risk prescribing.
• Reconcile medicines upon discharge from hospital and maintain continuity of supply for high‑risk groups.
• Signpost and refer patients to appropriate healthcare professionals in a timely manner.
• Manage repeat prescribing re‑authorisation, review medicines at review dates, and flag needed reviews.
• Conduct risk stratification of patient cohorts at high risk of harm from medicines.
• Develop pharmaceutical advice for new services, treatment pathways, and patient information leaflets.
• Analyse and present medicines data; lead clinical audits of prescribing; implement changes with the practice team.
• Implement changes resulting from MHRA alerts, product withdrawals, and national guidance.
• Monitor prescribing against local RAG lists and NICE guidance; provide newsletters on important prescribing messages.
• Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
• Work with the GP team to ensure compliance with CQC standards where medicines are involved.
• Support public health campaigns and provide specialist knowledge on available programmes.

We are the Leighton‑Linslade Health Connections PCN and are the primary care network for Leighton Buzzard. We work with three practices serving a population of approximately 52,000 and offer various primary care support roles to improve health outcomes.

• Minimum of 2 years post‑qualification experience.
