Family Centered Clinician

Summary

Provides service and develops treatment plans based on a person-centered assessment of individual strengths, needs, and goal orientation. Provides 24/7 on-call support for all assigned individuals. Builds and maintains positive relationships with community partners from referring or collateral agencies.

• Provide person-centered assessments in behavioral health, education, personality, community engagement, vocational needs, relationships, and family interactions.
• Develop and revise service or treatment plans based on assessment of strengths, needs, collaboration with referral agencies, and individual; goal oriented.
• Provide activity- and participation-based interventions and approaches derived from training materials, experience, and individual/family successes.
• Provide 24/7 coverage for all assigned individuals and/or families.
• Makes referrals to services and resources on behalf of individuals served and/or families.
• Ensure documentation and records are confidential, organized, complete, and submitted to supervisor accurately and timely.
• Participate in team meetings with peers and collateral team meetings when needed; support peer development.
• Provide discharge and aftercare plan for each family/individual served.
• Build and maintain positive relationships with community partners from referring/collateral agencies; guide families in interactions with referring agencies and other community organizations.
• May provide training or field supervision of new or tenured peers; review and edit peer records and documentation; conduct marketing and education presentations; training and development for region or team meetings.
• Perform other related duties and activities as required.

• Bachelor’s degree (or master’s in some states) in Social Work, Mental Health Counseling, Clinical Psychology, Counseling Psychology, or related field.
• One or more years related field experience; two years preferred.
• Specialized training required.
• Massachusetts:
  Must hold master’s degree in counseling education, counseling psychology, or rehabilitation counseling from an accredited institution; 2 years full-time supervised clinical experience in multidisciplinary mental health setting.
• Ohio:
  Must hold Bachelor’s (Master’s preferred) degree in social work or Master’s in counseling;
  Licensed Social Work (LSW), Licensed Practicing Counselor (LPC), or Marriage and Family Therapist (MFT) required.
• Driver’s license and acceptable driving record.

Light work; exerting up to 20 pounds of force occasionally, up to 10 pounds of force frequently, or negligible force constantly to move objects.
