Schedule:

Monday-Friday 8am-4:30pm, Day Shift, 40 hrs/week

Union: SEIU - Health Alliance Home Health & Hospice

Sign-On Bonus: $2,500

Responsibilities
  • Provides direct and indirect personal care in a safe and timely manner in accordance with departmental policies and procedures.
  • Performs homemaking and other environmental services in a way that facilitates sanitary and safe care in the patient’s home according to policies and procedures.
  • Duties may include:
    • Providing patients with mobility assistance, bathing, dressing, and grooming.
    • Providing patients and families with emotional support and instruction in areas such as caring for patient or self, preparing healthy meals, living independently, or adapting to disability or illness.
    • Changing bed linens, washing patients’ laundry, cleaning patients’ living area and other homemaking tasks identified within the home health plan of care.
    • Preparing or serving meals to patients according to the home health plan of care.
    • Directing patients in simple prescribed exercises or in the use of braces or artificial limbs.
    • Checking patients’ pulse, temperature, and respiration.
    • Changing clean dressings.
    • Performing a variety of duties as outlined in the home health plan of care.
  • Maintains records of patient care, condition, progress, or problems to report and discusses observations with supervisor or professional according to policy.
  • Observes, documents, and reports changes in client status or expectations of services promptly to a supervisor or professional according to policy.
  • Recognizes emergency situations and implements interventions according to appropriate departmental procedures and policies.
  • Renders services in accordance with the written plan of care as documented by the appropriate professional.
  • Position Qualifications License/Certification/Education
    • Current Home Health Aide certification or Certified Nursing Assistant (CNA) license.
    • Must have a valid US‑issued driver’s license and a registered, inspected, and insured automobile for work related purposes.
    • High School Diploma or equivalent.
    Experience/Skills
    • Minimum six (6) months of experience providing care as a certified home health aide in a home‑based setting.
    • Excellent oral and written communication skills.
    • Ability to read, write and speak clearly in English.
    • Ability to listen and give full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
    • Ability to develop and maintain constructive and cooperative working relationships with patients, coworkers, and supervisors.
    • Ability to consistently be responsible, compassionate, patient, emotionally stable, and cheerful with patients, caregivers, and other professionals.
    • Ability to be tactful, honest, and respectful of patients and family members in private homes.
    • Equivalent combination of education and experience may be substituted for the above requirements.
