• Provide home health nursing care to patients in accordance with the plan of care.
• Assess, plan, implement, and evaluate patient care to achieve positive clinical outcomes.
• Collaborate with interdisciplinary teams and families to meet patient needs.
• Maintain accurate documentation using electronic records in patient homes.
• Adhere to safety, privacy, and compliance requirements.

• Must have and maintain an active, unencumbered LPN/LVN license in the state of practice.
• Compact licenses must be transferred to the state of residence within 90 days.
• Current CPR certification (hands-on component).
• TB skin test current within last 12 months.
• Six months of hands-on nursing experience preferred but not required.
• Must have reliable transportation.
